Southwest Tennessee Long-Term Care Ombudsman ProjectSERVICES OFFERED: The Ombudsman Program is designed to ensure that residents of long term care facilities have a voice in their care and that their rights are protected. The Ombudsman receives, investigates and resolves a wide variety of concerns for residents and families of nursing homes, assisted living and board and care homes. The program also serves as a source of education for future long term care placement. Common issues addressed are: quality of care, financial information, resident rights and admissions and discharges. ELIGIBILITY REQUIREMENTS: Current or prospective residents of long term care facilities in 8 county area. INCOME/ASSET GUIDELINES: None SERVICE AREA: Eight Counties in West Tennessee: Chester, Decatur, Hardin, Hardeman, Haywood, Henderson, McNairy, and Madison. CONTACT: Amanda Scott at (731) 426-1312.
